day 5 family med

very interesting day in class today with this take home message:

by Dr. Norlehan qouted by someone i am not sure of. the verdict of my first percepting:

1. i talk too fast
2. i am too dominating to the patient
3. i have the self-confidence
4. i have to REVISE my BM
sigh~ huhu but shall be better in the future i promise!

lunch was infested with flies at the Ridz Cafe, watched Blue versus Green house ( 1-0 ) went coz did not want to sleep at home. and had dinner with bugsy at Dengkil!
have decided to stock food for ourselves at KK Dengkil coz we are always hungry there, i feel such joy clerking peads patients even when they cry. though i had to clerk and examine an antenatal patient ( spluttered and gagaped the whole way ) tak ingat how to!
i so have to study~

p/s: tak paham family med still balek lgie awl from ortho..but tommorow, no way!

family

General Practice, Chapter 2Characteristics of healthy families

  • Healthy communication. In this situation family members have freedom of expression for their feelings and emotions.
  • Personal autonomy. This includes appropriate use of power sharing between spouses.
  • Flexibility. This leads to appropriate 'give and take' with adaptation to individual needs and changing circumstances.
  • Appreciation. This involves encouragement and praise so that members develop a healthy sense of self-esteem.
  • Support networks. Adequate support from within and without the family engenders security, resistance to stress and a healthy environment in general. The family doctor is part of this network.
  • Family time and involvement. Studies have shown that the most satisfying hallmark of a happy family is 'doing things together'.
  • Spouse bonding. The importance of a sound marital relationship becomes obvious when family therapy is undertaken.
  • Growth. There needs to be appropriate opportunities for growth of individual family members in an encouraging atmosphere.
  • Spiritual and religious values. An attachment to spiritual beliefs and values is known to be associated with positive family health, supporting the saying 'The family that prays together stays together'

  1. Know yourself. The better you know yourself, the better you will know your mate. Learn about sex and reproduction.
  2. Share interests and goals. Do not become too independent of each other. Develop mutual friends, interests and hobbies. Tell your partner 'I love you' regularly at the right moments.
  3. Continue courtship after marriage. Spouses should continue to court and desire each other. Going out regularly for romantic evenings and giving unexpected gifts (such as flowers) are ways to help this love relationship. Engage in some high-energy fun activities such as massaging and dancing.

family medicine phew wit!

day 1: excitement!

reasons:

1. too much of herbalife tea in the morning

2. dapat famous amos and panda cookies in the morning!

3. interesting classes

the nicest surprise :)

started the day with cereal! in family medicine you HAVE to be punctual really or else u'll get a red mark on your logbook. mixed feelings today, sad to leave the relaxed life of ortho but looking forward to busy days. intro on FMS, dr azizah was true to what others described her to be, passionate with her work.
my first impression on dr hairel anuar ( name like papa ) is that he is a perfectionist. army styled hair, crisp shirt and shiny shoes, about 30 plus. dr norlehan is FUN! she conducted the lecture on the pillars in FMS and it was interactive. the first thing dr. siti told me when i entered class was:

'it is my personal aim to makesure u do not want to leave FMS by the end of the 8th week"

that's a wee bit scary, but who knew that nasal sprays, MDIs and peak flows would be interesting? a very simple yet educational CST with teaching on the proper way to explain patients when doing minute procedures. simple, yet important for thier well being.
all in all, a LOT of reading to do but a very good start i might say!

p/s: first day of official diet and saving money project. doing allright, set a deal with Tikah to join in too~

ortho final exam

meena was so scared she couldn't sit anymore

it started out with me waking up early to finish what i haven't studied yet and it ended up with me thinking hell i shouldn't have stayed up at all last nite or studied ortho!

let me enlighten u..

so i was the first person to be examined by Dr. Radhii ( the traumatologist who loves spinal surgery ) and i happened to get a case of cellulitis. the problem was this patient has DM, HPT and hypercholesterolenemia and history of infected bypass. i had so many things to ask but i dodn't have the time until he asked me to stop as i was about to enter the family history. so i already felt that i would fail. then for the physical examination he asked me to just do inspection.
the questions i got during the discussion were from BASIC medicine.

1. what happens during inflammation? what are the cell mediators and what does each of em do?

2. what is the first sign of nephropathy and connect it with bilateral leg swelling

3. what is SIRS ( systemic inflammation response syndrome and explain the pathophy?

4. what will u do if this patient comes to u at AnE?

5. what can we see at the x-ray and what antibiotics do we give?

so those are just the examples of the questions i unfortunately had to answer. scary! managed to crawl my way through with much difficulty. i can say this is not a gud way to start 4th year.

other than that, he was really nice and i can say for the future ortho students, he is unpredictable. he will just choose anyone.

hemi-thyroidectomy

p/s: not having second thoughts :)

so i am bloody tired, did a presentation bout Young Mercy will be presenting at the International Humanitarian Conference this thursday. went to an-nur to present to to prof ikram, ended up assisting in a hemi-thyroidectomy until 9pm. anuar intubated and assisted prof ikram. dr razale taught us step by step bout how to do a hemi-thyroidectomy. really cool.

i feel i am destined to become a surgeon, Insyallah.

what a day

sorry been out of contact, no internet in terendak and the battery for my handphone has been caput for two days. so a summary of terendak~

day2: clinic day first time met a person using a prosthetic limb ( he had a crushing injury 15 yrs ago ) so was fascinated. asam pedas melaka sgt sdap at sawit, but went tp pakcik dahans house and had SELEMBU ( seladang and lembu ). cried to sleep, first time i cried coz of u. met dr. rosman but dun minat him anymore.

day3: OT day: ACL reconstruction ( semitendonosis tendon ) and meniscus tear, arthoroscopy. lunch at pangkalan balak again and dinner at seri restaurant after playing futsal with the juniors and psychiatry students ( scored 4 goals in two games ). discovered Lachmans test is hard to do evn on kurus guys.

day 4: OT day: ACL resonstruction ( hamstring tendon ), wanted to watch a bone-tendon-bone but thin sgt, lunch at sawit ( rojak ayam sgt best ) and went to melaka town to eat dinner and jalan2 to jonker ( was closed ) and found that the parks were full of cute lighted animals. loads of fun. heard that some people xcukup elective credit hours, not me tho. phone habis battery.

day 5: last day. OT: crush injury ( open fracture of first phalanx ), lecture on blast injuries and mangled limbs. lunch at asam pedas sepang, went to seremban police station to see afeera and iman yg accident ( emergency break kereta afeera hit car infront ), back to dpp. slept a lot in the car.

currently: waiting for bugsy to take me home, sleepy.

another step in life

well i have great news!

this afternoon, kak yang gave birth to a healthy baby boy after just 3 hours labour! when she reached an-nur she was already 5cm dilated beb..so it was a normal SVD and both mum n son are healthy.

so, since one of us has taken the next step. when are the others gonna? huhu

other than that, i'm back in TERENDAK!! back to the sleepy resort rooms and great food! met Dr. Fuad today the one and only ortho. we had a gud start since the day started with a group breakfast at the usual place in front of the junction to the terendak camp [ i swear i'm gonna eat anything i want in malacca the food is so good ] then we had a briefing and straight away clerked the patients. there are loads actually in comparison to IM dulu. we have two patients each and tommorow we'll start presenting! wuhoo!
the cases are also quite interesting, a lot of sports injuries like meniscal and ligament tears but we also have cellulitis and OA cases as well. and the patients here are superbly nice.

but the best thing is that now we are in fourth year and have adapted to clinical life, we find it no problem to present esp. at terendak. the first thing we actually did when we arrived was to introduce ourselves to the staff and went to clerk so i think we really did have a good start. plus we went to the OT to watch a CMR on a 5 yr old boy who obtained a supracondylar fracture. all this while we have been watching CMRs done but we really did not know how to do it and what to look for while doing it so today we learned.

THE FOOD!! so what we did was go to pangkalan balak today for lunch [ 7 org share ABC! ] and then we went to Sawah restaurant for dinner [ the one with the green nasi goreng ] i can't wait for the goreng pisang and rojak for tommorow. we are going to Sawit pulak!

p/s: the downsides today are that i didn't see Dr. Rosman and the internet here kinda sucks
